Repairs and Returns

When sending items to Mizuho America for Repairs and Returns, follow the steps below:

Request RGA number by completing the form below. You will receive an RGA number via email within 24 hours.

Required *

Certificate of Sterilization must be filled out by hand and included in the shipping container or faxed to 510-324-4545. Items being sent to Mizuho America will not be processed until this document has been received and deemed satisfactory.

Package the item being returned properly to prevent additional damages that may occur during shipping. Clearly identify RGA number on top of the shipping container.
